DOS ain't dead

I pulled the old instruction reference (mostly listing 686 level instructions) from NASM sources and, after verifying that this is allowed, provide it under the new NASM license (2-clause BSD).

A live hosted version of the HTML (single-page) output is available at https://ulukai.org/ecm/insref.htm This is updated by a cronjob on the server, so sees any updates I upload to the repo.

The repository is at https://bitbucket.org/ecm/insref/ (This is currently missing hg tags created from the original git tags, because of a difference in these SCMs.)
